The California Lab 182 form is an application for certification as a phlebotomy technician in California. It is required by the Department of Health Services and must be completed to obtain certification in one of the designated categories: Limited Phlebotomy Technician (LPT), Certified Phlebotomy Technician I (CPT I), or Certified Phlebotomy Technician II (CPT II).
You must submit the following documents with your application:

What is the processing time for the application?
Processing your application can take at least 60 days. This time frame begins once the Department of Health Services receives your fully completed application along with all required documents.
What happens if I submit an incomplete application?
If your application is incomplete, it will not be evaluated. Ensure that all sections are filled out and that you have attached all necessary documents to avoid delays.
Can I apply for multiple certification categories at once?
No, you must check only one certification category on the application. Choose the category that best fits your qualifications and training.
What if I have a criminal record?
If you have been convicted of any felonies or misdemeanors (excluding minor traffic violations), you must disclose this on the application. Attach a statement providing details about the conviction. Failure to disclose this information may affect your eligibility for certification.
